Cost Per Admission, defined

Short answer: Cost per admission (CPA) is total marketing spend divided by admitted patients, calculated overall and, more importantly, by channel. It is the only marketing metric that connects advertising to the P&L: cost per lead and cost per call reward volume, while cost per admission rewards revenue.

Why It Matters

Why it matters

Channels routinely rank opposite on cost per lead versus cost per admission, cheap leads that never admit are expensive. CPA by channel is what lets a center cut losers, scale winners, and defend the marketing budget to ownership in revenue terms.

Measurement

How it is measured

Spend ÷ admits, per channel, on cohorts (admits lag inquiries by days to weeks). Requires the attribution chain: call tracking with DNI, CRM source stamping through admit, and a dashboard joining spend to outcomes.

Benchmark

Benchmark and example

A defensible CPA is a percentage of revenue per admit, commonly targeted in the 5–15% range, not a universal dollar figure: a $30K residential admit supports a far higher CPA than a Medicaid outpatient admit. Judge each channel against your own revenue per admit.

Common Mistakes

Common mistakes

  • Judging channels on cost per lead or cost per call
  • Using blended CPA, which hides losing channels
  • Judging on calendar weeks instead of cohorts
  • No offline conversion feedback, so ad platforms optimize to calls, not admits
  • Ignoring conversion leaks: a broken funnel inflates CPA no budget can fix

Improvement

How to improve it

Build the attribution chain, cut channels with indefensible channel-level CPA, feed admit events back to ad platforms as offline conversions, and fix answer/VOB leaks, conversion repairs lower CPA without touching spend. The budget guide has the math.

FAQ

Questions, answered.

What is a good cost per admission for a treatment center?
A defensible share of that admit’s revenue, commonly 5–15%, which varies by payer mix and level of care. There is no universal dollar benchmark.
How do you calculate cost per admission by channel?
Channel spend ÷ channel-attributed admits, on cohorts, requiring call tracking, CRM source stamping, and a dashboard that joins spend to admits.
Why is CPA better than cost per lead?
Because channels invert between the two: the cheap-lead channel is often the expensive-admit channel. CPA is the number the P&L feels.
